This program gives you the tools needed to close your checking account with another financial institution, move your direct deposit and reassign your automatic payment - all without even leaving this site!

Simply click on the links below to access the forms you need to get started.  (Note: select “landscape” as your page orientation when printing these forms)  It's really as easy as 1-2-3!

1. Checking Account Closure Notification - Informs your current financial institution that you will be closing your account and where to forward your money at that time. When the form is completed, mail it to the institution where you are closing your account.
   
2. Direct Deposit Change Notification – use this form to notify an employer of a change in financial institution for direct deposit or payroll deduction purposes.
   
3. Automatic Payment Change Notification - Use this form to inform companies of your new checking account for automatic transactions (any recurring automatic payments like internet access, gym membership, car payments, etc.).
